## How To Enroll

Contact Nationwide at 1-877-738-7874 and provide them with your NVIDIA [Alternate ID](https://www.nvidia.com/en-us/benefits/health/alternate-id.md) or enroll online at <http://www.petinsurance.com/nvidia>.

If you sign up for payroll deductions between the 1st and 15th of the month, your plan will become effective on the 1st of the following month.

Example:

Enrolled May 1 = June 1 effective date

Enrolled May 16 = July 1 effective date

Cancel enrollment anytime by calling Nationwide. Depending on the timing of cancellation, you might still see a deduction from your paycheck. Nationwide will automatically issue a refund on any unearned premium once the deductions are received.
